If you are having trouble with the online portal, you can print the New Patient Forms as a PDF with the link below. Please fill out these forms and bring them with you to your initial appointment . If you have any questions, please fill out as much as you can and our staff will be happy to assist you when you come to the office.

Prescription refill requests can be done through your pharmacy. Typically, the pharmacist will fax our office a request to refill the prescription. Please note that this process may take between 3-5 business days to complete, so please plan accordingly. Due to regulations, we will not be able to refill medication if we have not seen you in the office in over 1 year.
